What are the resonsibilities of Hearts & Hands Of Care, Inc.?

  • Liaison between the Division of Senior and Disability Services and the consumer
  • Processing payroll for the PCA including withholding taxes
  • Providing workers' compensation coverage and unemployment taxes
  • Consumer education on all CDPC policies
  • Developing individual service plan with the consumer
  • Review of backup, training, and recruitment plans
  • Providing resources to encourage self-advocacy
  • Submit and review state and federal criminal background checks for PCAs.

What are the responsibilities of the customer?

  • Participate in developing your service plan with Hearts & Hands Of Care Inc.
  • Recruit, train, schedule and manage all assistants who will provide your personal care services
  • Approve all Service Delivery Records which will be submitted to Hearts & Hands Of Care Inc.
  • Resolve any PCA issues which may arise
  • Develop an emergency backup plan
  • Notify Hearts & Hands Of Care Inc. of any changes in condition, living situation, or attendant services.

 

 What is the Consumer Directed Personal Care Program?

CDPC is a Medicaid entitlement program which allows a person (consumer) with a disability to recruit, hire, train, and manage a personal care attendant of their choice. The consumer is able to train their Personal Care Assistant (PCA) to meet their specific health care needs and maintain a schedule convenient and flexible to both the consumer and the PCA.

  • Ability to direct yoru own care, or to appoint someone who can

  • to have a single point of contact for information and advice on the full range of services and other assistance available in their area to have access to coordinated services relevant to their situation
  • for volunteers to be able to participate in volunteer networks to provide support to careers

For the wider community

  • to better understand who care givers are, to better understand care givers' stress and the appropriate use and availability of services and supports

Hearts & Hands of Care

  • to be well coordinated around, and responsive to the needs of, care givers, in particular
  • achieving better matching of services to the individual needs and circumstances of care givers and care recipients
  • ensuring equitable access for care givers across and within regions to a range of services offering services in different settings, including in-home settings and out-of-home settings; and improving care givers' access to services on a planned basis or in emergency or unplanned situations
