Through the funding sources of Medicaid Waivers and grants, Hearts & Hands of Care, Inc. provides support catered to individuals and families. Habilitation serves people of all ages who receive services through the following types of waivers:

  • CCMC — Children with Complex Medical Conditions
  • MR/DD — Mental Retardation / Development Disabilities
  • APD — Adults with Physical Disabilities
  • OA — Older Alaska

Waivers provide access to many care services that include:


Daily and Hourly Respite services are designed to provide short term relief from caring for a family member with special needs. This service is available for unpaid care providers (parents and guardians) and Assisted Living Home operators. This service helps prevent burnout from the everyday demands of care giving.


Supported Living service assists and encourages adults to live independently in their own home and learn daily living skills.


In-Home Support assists individuals in increasing independence through home based goal work.


Family Habilitation support provides daily assistance in a licensed Assisted Living Home and family like setting.


Day Habilitation includes community outings and activities accompanied by the Hearts & hands staff.


Nursing Intensive Active Treatment service available for monitoring and assessing the needs for children with acute medical conditions.
