Care coordinators assist individuals and families with Medicaid waivers. Our Care Coordinators assist people with Medicaid waivers and teach them about choosing the right services based on their needs and goals. Once the plan for services is in place, Care Coordinators help the family put it into action and monitor implementation.

 

We provide a basis for mutual self help and support for families where there is a family member or individual with a disability, in order to relieve some of the stresses on the family that arise with a disability.

 

Our aim is to reach mutual support, gain information, advocacy, public participation, access and prevention. 

 

Our objectives include initiating and maintaining contact with families, creating schedules for parents, sharing information, education and participation with other agency networks and government agencies. 

 

People we serve include two parent families, single parent families, individuals with disabilities, individuals that are rurally located, children, adolescents and senior citizens.

 

We help families brainstorm solutions to problems, help them navigate the complex social service system, assist in locating medical and adaptive equipment. We assist with applications for services that include: DD Eligibility, TEFRA, Respite, SSI/SSA, Transportation and Public Assistance.
